As the parent or legal guardian of the above named minor, I give my permission for him or her to participate in the 2nd-4th Grade camp at Maranatha Bible Camp June 26-June 28, 2025. I give my permission for the above named minor to ride the bus to and from Maranatha Bible Camp.

As the parent or legal guardian of the above named minor, I give my permission for him or her to participate in activities, events, and programs of Christ’s Church of Oronogo during the year September 1, 2024, through August 31, 2025. I understand and grant permission for my child to participate in activities during the event including zip lining, archery, swimming, inflatables, as well as other indoor and outdoor recreational games and activities. I understand the inherent risks that are involved in these activities and hereby release CCO, its staff, employees, and volunteers from the responsibility and liability for any injury or illness sustained during these activities, events, and programs.

Further, I do authorize the minister, adult leader, or sponsor of the activity, event, or program, or any CCO staff member, in the event I cannot be reached by phone, to give consent to a physician and/or hospital for emergency medical or surgical treatment. It is understood that I will assume any financial responsibility for any expense that may be incurred for said emergency treatment.

Camper Behavior: I recognize this is a Christian camp, that the Bible will be studied, and that conduct will be expected that is consistent with Biblical values.

Medical Treatment: To the best of my knowledge, the participant on this form is physically and emotionally able to take part in the camp program. In the event of a medical emergency, I give permission to the volunteer medical person selected by the camp management to do whatever is necessary for the health of the participant named on this form. I understand the Maranatha Bible Camp staff have preventive measures in place to reduce the spread of communicable diseases including COVID-19 but they cannot guarantee that an event participant will not become infected with COVID-19 or other communicable disease. I hereby release the camp from any responsibility other than normal supervision and care. In case of an illness or accident, I will not hold Maranatha Bible Camp or its staff, management, or officers liable.

Photo Permissions: I give my permission for appropriate photographs and videos to be used for future Maranatha Bible Camp promotional materials.

I understand that participants attending Maranatha Bible Camp may be offered an opportunity to participate in multiple recreation options including but not limited to: Swimming, Riding Zip Lines, Archery, Human Soccer, Axe Throwing, and playing on Inflatables. I fully understand and acknowledge that 1) risks and dangers exist in the participation in and use of these recreation options, 2) that participating in such activities and/or use of such equipment may result in injury or illness including but not limited to bodily injury, disease, strains, fractures, partial and/or total paralysis, eye injury, blindness, heat stroke, heart attack, death or other ailments that could cause serious disability; 3) and by giving permission for the person named in this registration to participate in these activities and use this equipment, I assume all risk, dangers, and responsibility for any losses and/or damages incurred while participating in these activities. I release Maranatha Bible Camp and any entities they contract with for specialized recreation from all liability. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of this organization, its employees, agents, and representatives, whether an injury or infection occurs before, during, or after participation in any such program, service, or activity.
